Daily shift report templates help an incoming team understand what changed, what was completed and which items still need attention. Define the reporting period, use factual observations and name the owner of each next action. A handover should summarize the shift while keeping detailed job records easy to trace.
Use a daily activity log for time-based entries, an attendance sheet for staff presence and an action plan for continuing improvements. Reference these records in the handover without treating presence as hours worked or a planned action as completed work.
This page now includes two original downloads: Daily Shift Handover Summary and Shift Open Items Register. Choose the Daily Shift Handover Summary for a concise account of one shift. Choose the landscape Shift Open Items Register to carry unresolved items across several handovers.
The new files appear first with previews and clearly labeled download buttons. The original gallery and its resource examples follow the practical guide; descriptions of the older collection refer to that original gallery. The new PDFs have fillable fields; save a local copy, open it in a compatible PDF viewer, and review the saved result before printing or sharing.
Keep this article focused on the transfer of work between shifts. Use a work order for a particular assigned job, and reference that job number in the handover when it remains open. A maintenance request can record a newly reported equipment or facility issue without turning the shift summary into an unapproved repair instruction.

How to use the new downloads
Complete the shift start and end details, then distinguish completed work from outstanding work and dependencies. Record the receiving lead’s actual handover confirmation rather than assuming a saved report was read. In the register, give each open item a reference, current state, next action, owner, and due information.
If a fictional equipment inspection is waiting for access, keep that condition visible instead of marking the task completed. The incoming owner can record the next action while retaining the original item reference. Urgent incidents and specialist concerns should follow the workplace’s established process; a handover form does not supply an emergency or clinical protocol.
What Is a Daily Shift Report?
A daily shift report is a written record of the activities, events, problems, and results associated with a particular work shift. It provides continuity when one group of employees finishes work and another group takes over.
A shift handover report can explain what has already been completed, what remains outstanding, and whether there are issues requiring attention. This makes it easier for the incoming team to continue work without repeatedly asking the previous team for basic information.
What Should a Shift Report Include?
The information included in a daily shift report form depends on the industry and type of work. A typical report may document the shift date, employee or supervisor information, work completed, pending tasks, production or service results, equipment problems, incidents, customer issues, safety concerns, and instructions for the next shift.
The most effective report is not necessarily the longest one. It should capture the information that the next employee, supervisor, or manager actually needs to understand the status of work.
Daily Shift Report Template for Business
A daily work shift report template can help businesses establish a consistent reporting process. Managers can use the same format for each shift, making reports easier to review and compare.
Manufacturing companies can record production output and machine problems, while restaurants can document customer activity, staffing issues, inventory concerns, and important events. Construction supervisors can use a construction daily shift report to record completed work, site conditions, delays, equipment issues, and safety observations.
Shift Handover Report
One of the most important purposes of a shift report is communication between outgoing and incoming employees. A shift handover report provides a written summary of information that needs to be transferred from one shift to another.
A good handover report should make outstanding tasks and important issues easy to identify. This can reduce communication gaps and help employees begin their shift with a clear understanding of current priorities.
Free, Editable and Printable Shift Report Templates
An editable daily shift report template can be customized for your organization by adding company information, department names, employee fields, reporting categories, and other workplace requirements.
A printable shift report template is useful for offices, factories, construction sites, restaurants, warehouses, and other workplaces where employees may prefer completing reports on paper. A digital version can also be maintained when your organization uses electronic reporting.
Using a standardized daily shift report form can improve accountability, preserve important information, and make communication between shifts more consistent.
Set a reporting period that can be understood
Write the shift’s start and end dates and times, the location or department, the outgoing team, and the person receiving the handover. A shift that crosses midnight needs both dates. A report labeled only “Tuesday night” can become ambiguous when staff review it later or compare it with a job record.
Separate activity totals from open issues. Production output, customer requests, completed dispatches, and unresolved work do not belong in one unlabeled number. Define each measure and its unit. If the report counts completed orders, do not also count the same partly packed order as complete simply because it was handled during the shift.
State what is known at the reporting cutoff. Work can change after the outgoing person writes the draft but before the incoming team assumes responsibility. Update a material change or explain the later event through the established process. The record should identify the status being transferred rather than present an earlier draft as the final position.
Write open items with a next action
For each unresolved item, record the reference, current status, next action, owner, and relevant timing. “Supplier issue” is less useful than “Order PO-220 is awaiting supplier confirmation of the remaining four trays; purchasing will check the reply before the next dispatch review.” The second note tells the next team what remains uncertain and who is handling it.
Keep a reported symptom separate from a confirmed finding. “Printer produced a paper-feed error at 18:20” describes an observed event. “Printer motor failed” asserts a cause that may not have been established. Record the relevant work-order or request reference and the actual operational status under workplace procedures rather than making a diagnosis in the handover.
A shift report does not itself authorize maintenance, change a safety control, or replace an emergency communication route. Follow the established process for urgent issues when they arise; use the report to reference what was communicated and what remains open. The person receiving the summary should not have to discover a critical issue buried among routine totals.
UK HSE guidance describes preparation, the exchange of relevant information, and cross-checking by incoming personnel as parts of effective handover. It also recommends both written and verbal communication. Use the report as support for that exchange rather than assuming that saving a file proves another person received and understood it.
Fictional example: a warehouse shift handover
Consider an invented shift from 14:00 to 22:00. The team completed 26 dispatch orders and left two awaiting required packing details. The outgoing lead records the two order identifiers and the missing information, then identifies the incoming lead who will review the replies. The handover does not include those two orders in the completed total.
A delivery of 12 cartons was recorded during the shift, but the receiving check for one carton remains open. The report references the receiving record and identifies the unresolved check. It does not state that every item was accepted or copy an assumed stock balance into the summary. The underlying receiving and inventory records retain those detailed facts.
A label printer showed a fault message at 18:20. The team followed its approved reporting route and referenced maintenance request MR-207. The handover states the actual status recorded under that procedure and the named contact for the next update. This example illustrates documentation, not instructions to operate or repair the equipment.
At handover, the incoming lead reviews the two open orders and the request reference with the outgoing lead. If a question changes the understanding of an item, the report is clarified before the handover record is finalized. Acknowledgment of receipt remains separate from closure of the underlying jobs; reading the report has not completed those tasks.
Keep summaries concise and supporting records connected
Use the to-do list for routine actions that need an owner and follow-up date, and reference those actions from the shift report where useful. Keep detailed customer messages, inventory transactions, and maintenance findings in their appropriate records. The shift report should summarize the state of work and point to the detail, not become a duplicate of every record created during the day.
For hotel or reception work, the same principle applies to pending guest requests and service follow-ups. Record the reference, status, and responsible person through the organization’s approved process, with only the information the next team needs. A generic report is not a suitable place to reproduce unrelated private customer information.
The nursing-related resource has a specialized purpose. Clinical handovers require the organization’s approved clinical records, procedures, and trained staff; these generic examples do not supply clinical guidance. Likewise, a site with safety-critical work needs its own approved reporting and handover requirements. Adapt the layout within the real process rather than assuming the sample establishes it.
Review the handover before filing it
Check dates, references, quantities, ownership, and any statement that an item is complete. Resolve inconsistent numbers or explain a known uncertainty. If the same job appears in two sections, make the relationship clear so a reader does not interpret it as two separate tasks.
Record the actual handover event according to workplace procedures. A prefilled name is not a real acknowledgment. Preserve the finalized shift record and add later corrections transparently through the approved process rather than silently changing the history. Keep the blank master separate from completed reports so the next shift does not inherit old results or acknowledgments.

Frequently Asked Questions
What formats are the two new downloads?
The two new files are fillable PDFs with different purposes and matching previews. Open them in a compatible PDF viewer, complete the fields and save your copy. They are not DOCX files, and numerical PDF fields do not calculate totals automatically. The original gallery has its own mixed resources and formats.
How should I label a shift that crosses midnight?
Record both the start and end dates and times. Include the location or department so the reporting period is clear when the record is reviewed later.
What should an unresolved task entry contain?
Give its reference, current status, next action, owner, and relevant timing. A short actionable entry is more useful than a vague description of a problem.
Does a saved report prove that handover occurred?
No. Follow the actual communication and acknowledgment process. A written record supports the exchange but does not establish that another person received and understood it.
Should pending orders be counted as completed?
Define the measure and count according to that definition. Keep unfinished orders visible as open items instead of including them in a completed total.
Can the report authorize equipment repair?
The shift summary is not an authorization process. Reference the appropriate request or work order and follow the workplace’s maintenance and safety procedures.
Can the nursing example be used as clinical guidance?
No. Clinical handovers need approved organizational records and procedures used by qualified staff. A historical layout does not provide clinical instructions.
How do I correct a finalized report?
Use the workplace’s approved correction process and preserve the history. Avoid silently overwriting the original event, ownership, or completion statement.
